Care Awards Nominations Now Open for 2017!

The annual Care Focus Care Awards will be held on Friday 7th April 2017 at Wellsprings Leisure Centre in Taunton.

This will be 10th year of Care Focus running this important annual event and we hope to have even more nominations this year.

The awards are open to all individuals involved in care, support, health and housing either employed or voluntary. All entries are treated confidentially.

There are 12 categories for nominations:

  • New to Care Award
  • Outstanding Volunteer / Unpaid Carer Award
  • Unsung Hero
  • Outstanding Ancillary Worker
  • Outstanding Commitment to Learning and Development
  • Quality Specialist Award
  • End of Life Care Award
  • Excellence in Leadership Award
  • Outstanding Care / Support / Healthcare worker – Tim Catley-Day Award
  • Outstanding Care Team
  • Care Organisation of the Year
  • Outstanding Contribution Award

How to submit a nomination

  1. Download the Nomination Pack (PDF)
  2. Choose the category or categories for which you want to nominate your colleague or yourself. If you are entering more than one category, separate entry forms must be completed. Read the judging criteria carefully
  3. Download the Nomination Form (PDF). Right click the blue link and "Save As" to your documents folder.
  4. Write an account of no more than 200 words saying why your colleague (or you) should be considered. An additional account from a colleague or service user/family carer is helpful but not essential.
  5. Please post printed entries with supporting testimonials to Care Focus (SW) CIC Unit 7, Smalls Yard, Dellers Wharf, Taunton, Somerset TA1 1NU or digital entries can be emailed to enquiries@carefocussw.co.uk.
  6. Closing date for entries - extended to 2nd December!

HELP & GUIDANCE ON COMPLETING NOMINATIONS

  1. Please make sure that the name of the nominee and the award category is included on all separate pieces of supporting information.
  2. Complete the nomination form and attach it to all other separate pieces of supporting information.

Rules for entry

  • The awards are open to all individuals involved in care, support, health and housing either employed or voluntary. All entries are treated confidentially
  • Entries will be judged by a panel of reputable, independent and impartial adjudicators
  • Entries will not be returned
  • One copy of the submission is required
  • Use one entry form per entry. Photocopies are acceptable.
  • All finalists will be expected to attend a photo-shoot held week commencing 6th March 2017, venue to be confirmed.

IMPORTANT

  • A statement of up to 200 words saying why you think the nominee should win the Award should be included.
  • Please only submit 200 words – no other supporting materials will be considered except testimonials.
  • Supported statements from a colleague/service user/family carer may be included but are not essential.

Finalists will be announced during January 2017. The awards will be presented at a Gala Dinner on Friday 7th April 2017 at the Wellsprings Leisure Centre, Taunton.
